Granting financial help for
infertility treatments …fulfilling dreams….building families
ABOUT THE
GRANT
Baby Quest will
select applicants who have medical insurance but who are
uninsured for fertility treatments. The grant can be used
towards IVF or IUI cycle, an egg donor, or to assist with
the cost of surrogacy (within the United States ONLY.)
THE PROCESS:
Grant(s) will be
awarded two times yearly. The application deadlines are
generally May and November. (Specific deadlines can be found
on the application form.) Finalists will then be
contacted to provide additional information such as proof of
income with documentation (copy of last two IRS tax returns,
etc.). Recipient(s) will be notified approximately two weeks
after the application deadlines.

Written
description of fertility history (photos - optional but
welcome)

Letter from
a Reproductive Endocrinologist and/or Infertility
Specialists (REI) stating fertility history and
suggestions for specific procedure (applies to female
applicants only). Male applicants using own sperm must
submit documentation regarding own reproductive health.

Signed
Medical form – must be signed by applicant and physician
(see online application)

Signed
release form (see online application)

A copy of
both sides of applicant’s insurance card and that of a
partner (if applicable)

Application
fee of $45. (Please make check payable to Baby Quest
Foundation.)
If chosen:
Background check ONLY if chosen (Cost to be paid by
recipient.) Signed copyright and media release giving Baby
Quest full rights to your story beginning immediately upon
selection.
We welcome
applications sent via regular, priority, or express mail.
Please do NOT send packet with signature required. This
creates a delay. We will confirm receipt of your application
by e-mail.
