Application Process

Grants will be awarded two times yearly. The applications MUST be in our hands by the deadline. Finalists may be contacted to provide additional information. All applicants will be notified via email approximately four to six weeks after the application deadline. Please make sure your computer is set to receive emails from Baby Quest Foundation.

   NEXT GRANT DEADLINE:   September 5, 2019

Baby Quest receives hundreds of applications and we are limited by the funds that are contributed. Please know that, as much as we want to fund all who apply, this is NOT possible.

Because funding is limited, we do NOT award grants to those already receiving funding from other organizations. (This does NOT include bank loans.)

To ensure privacy, applications of those not selected as grant recipients will be destroyed one month after the grant deadline.


  • Completed Baby Quest application (See below to download)
  • Background check only if selected. (Cost of approximately $250 to be paid by recipient)
  • Personal  story – 2 pages maximum (photos optional but encouraged)
  • Signed media release form giving Baby Quest permission to use your story (included with application.) Recipients are often asked to do interviews, both print and tv. This is required.
    Photos will also be posted on Facebook with first names and location only.
  • A copy of both sides of applicant’s insurance card and that of partner (if applicable).
  • Application non-refundable fee of $50. (Please make check payable to Baby Quest Foundation.) (Checks only no money orders please.)
  • Completed Medical Packet. This portion of the application is to be completed by the physician.
  • Please allow sufficient time for your physician to complete these pages and return them to you. Submissions must include these forms to be considered complete.
