Eligibility Checklist
Type of treatments/procedures covered: This Perelel IVF Grant funds in-vitro fertilization and additional costs associated.
Failure to conceive: Baby Quest covers the following ASRM definition of infertility – “The need for medical intervention, including, but not limited to, the use of donor gametes or donor embryos in order to achieve a successful pregnancy either as an individual or with a partner.” (ASRM, 2023).
Overall health: All applicants must demonstrate overall good physical and mental health and have no medical conditions that would pose a serious risk to themselves or, in the case of females, the child they would carry (e.g. active acute infections).
Residence: Applicants must be permanent residents of the United States and must be under the care of an accredited fertility facility (of their choice) within the United States.
Living arrangements: All applicants must maintain adequate living arrangements (must lease or own a residence).
Means for child support: All applicants must possess the means to offer continued child support (at least one partner must maintain employment).
Health insurance: All applicants must have basic health insurance for mother and baby with sufficient prenatal coverage. We generally don’t fund those with extensive fertility insurance coverage.
Your fertility timeline: Baby Quest does not reimburse for procedures already begun.
Readiness to begin treatment: Recipients must begin treatment within three months of receiving a grant.
Diversity: We respect diversity. Grant is available to same-sex and heterosexual couples and singles.
DEADLINE: MAY 15, 2024

While all 17 sections of the online application must be completed, here are some important items that will be required:
Medical evaluation form: To be completed by the physician. Please allow sufficient time for your physician to complete these pages and return them to you. You will be asked to provide the items below when you complete the application online.
Personal story – 1000 word maximum. Photo. Example of a personal story
Signed media release form giving Baby Quest permission to use your story (included with application)m. Recipients are often asked to do interviews, both print and TV. Photos will also be posted on social media with first names and location only. Baby Quest firmly believes in destigmatizing infertility. Our recipients must be candid about their fertility experiences.
A copy of both sides of applicant’s insurance card and that of partner (if applicable). We generally don’t fund those with extensive fertility insurance coverage.

- When will Perelel and Baby Quest make a final decision about the IVF grant? The week of May 20th.
- Is there any cost to me? Perelel will cover up to $15,000 to put towards your IVF round. Total cost of your IVF round and associated costs may vary based on your specific case.
- If I am selected for the IVF grant, is there a specific timeline I would need to plan for? If selected for the grant, you would need to start your procedure within three months.
- Will my application be kept anonymous? The mission of Baby Quest is to destigmatize infertility. We will not publicize that you applied. However, if you apply and receive the grant, you must agree to the promotion of it by Baby Quest.
- Is this open to all genders? Yes—and singles, too.
- Can I apply on behalf of a friend? Sorry, no—a medical evaluation form is required to be completed by their doctor, so it is best that they apply personally.
- Can the grant be used for IUI? No, the Perelel Grant cannot be used for IUI.
- Can the grant be used for egg freezing? No, the Perelel Grant cannot be used for egg freezing.
- Can the grant be used to pay for surrogacy? Yes, but the grant will only cover a portion of those costs as it is $15K.
- Can the grant be used to pay for donor eggs/sperm? Yes.
- Do I need to supply personal medical information in the grant application? How will that information be used? (Thinking about HIPAA/privacy here…) There is a HIPAA statement to sign included in the application, and a form that must be completed by your reproductive specialist.
- How will the recipient be selected? Baby Quest’s committee of women’s health advocates, financial advisers and a medical team will review applications. They will select a recipient.
- If I am not selected this round, will there be more grant opportunities in the future? Baby Quest awards grants twice yearly. You can learn more on https://babyquestfoundation.org
- Where does the money go once a recipient is selected? Baby Quest will directly cover $15K of costs associated with the clinic, attorney, lab, genetic company or etc. When payment is needed, Baby Quest provides a credit card for the services rendered.

ANNOUNCEMENT OF THE PERELEL GRANT RECIPIENT:
We plan to make an announcement the week of May 20th! Those NOT receiving a grant will receive an email. Grant selection will also be announced on social media.
